If you want to join the practice please complete a registration forms GMS1 registration form (PDF, 156kb) which can also be picked up from reception. 

Please bring the completed forms along to reception. 

For children under 6 years of age, please bring their RED book to allow us to check their immunisation status.

When you have completed and returned the form and you have been accepted as a patient, , NHS England will transfer your medical records to us and write to you to confirm your registration as a patient with that practice.

You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
